如何取得憂鬱症康復證明

小弟2006年818醫院停役後覆診一年以取得免役證書

6年來很久沒回去看診拿藥

我最近好像查到了有康復證明這種東西好像些單位或考公職需要用到想拿當初拿到

免役令後就沒去管他所以沒有請醫生開給我

然後上網查到有免役令就不用擔心回役問題

要如何取得?是回去818看診叫醫生開給我嗎?

可是怕回去看診6年沒就醫的記錄會斷有些保險公司說好像要有5年內沒有看診記錄

去拿康復證明算回去就整記錄嗎??

有人有這張康復證明的證書嗎?請問你們是如何拿到的?
