薪資條上面細項如下
基本薪資 17000
伙食費 1470
加班費8450
過定價日工資 973
全勤獎金1000
休假到工2000
激勵獎金1500
紀律獎金1000
代扣金額
勞保費378
健保費309

應發金額33393
實發金額32706

Q1以上是否違反勞基法最低薪資?(煩請附上計算式)
Q2投保金額最低應為多少?
Q3從以上的加班費是否推算的出有過時加班之嫌?
Q4 另 公司不肯提供上下班打卡紀錄'(刷卡方式)無法證明是否超時加班，如何處理?
PS 當事者上班時已經疑似過勞 心肌梗塞 休克 腦缺氧 四肢癱瘓 幫忙詢問的 !
