改名需要哪些證件

我過幾天要改名字
有先規劃好程序，請各位大大看這樣可不可以
也請你們提供一下到底要什麼證件，打電話問了還是不懂= =

戶政事務所(身分證)→健保局(健保卡)→富邦產險(強制責任險)→監理站(行照+駕照)→郵局→亞太、中華

身分證用的照片有4張，兩吋照片有8張，這樣應該夠用吧- -?
新印章我也刻好了!
個人戶籍謄本申請10份會不會太多啊?
辦理上面那些，當天會拿到證件嗎?!

因為我在外地工作，所以這些我要一天就弄完...
工作的則是回台中在去更改
