身分證遺失該如何補辦及應備證件?

一、辦理遺失國民身分證應由本人親自向戶籍地戶政事務所申請。
二、應備證件為本人印章（或簽名）及2年內彩色相片1張，若相片掃瞄建檔取像日期在2年內，經戶政事務所核對人貌相符者，得免繳交相片，直接列印該檔存相片製作國民身分證；另核對當事人容貌產生疑義時，應查證其他附有相片之證件或相關人證等方式，以確定身分〈政府機關核發貼有相片之有效證件：護照、10年內畢業證書、退伍令、健保卡等〉。
三、身分證補領規費新臺幣200元。
